Why and how does this happen?

So, I was scrolling through old forums (because I'm weird.), and noticed this:

(In case image thing isn't working, it's an image of a topic DUPLICATED. It's two topics that are IDENTICAL but they have different Last post by —'s, or whatever.

So, can someone explain to me why or how this happens?

Maybe because someone made it twice?

Lag, it happens a couple times. Mainly with quoting but sometimes topic creation.

Lag, it happens a couple times. Mainly with quoting but sometimes topic creation.
That, or the user was intentionally spamming it. If the thread itself seems relatively normal, it was likely lag.
