Can I use "Scratch" in the name of a derived app?
I've been a bit confused here and I'm just making sure since, even though there are extensions and related applications that use “Scratch” in their name, there seems to be a trademark policy that prohibits using “Scratch” to endorse apps derived from Scratch's software: https://github.com/scratchfoundation/scratch-gui/blob/develop/TRADEMARK Note that my app doesn't intend to make any profits
(by the way, the policy still states that Scratch is property of MIT. this probably needs to be updated right?)

According to the Terms of Use:
Scratch ToUIt seems a lot like no, but I'm not sure how the extensions can use it.
5.4 The Scratch name, Scratch logo, Scratch Day logo, Scratch Cat, and Gobo are Trademarks owned by the Scratch Team…
Unless you are licensed by Scratch under a specific licensing program or agreement, you may not use these logos to label, promote, or endorse any product or service.

It seems a lot like no, but I'm not sure how the extensions can use it.There may be a difference between “You're not allowed to use it” and “The Scratch Team actively tries to stop those who use it.” Well, really, “You're not allowed to use it” is always correct. But “The Scratch Team actively tries to stop those who use it” is not always correct, perhaps.
Legally, I don't think you should use the name “Scratch” to label/promote/endorse any product/service. Whether or not you would be legally pursued for using it is up to the discretion of the Scratch Team. If you want to be 100% safe, don't use the Scratch name.
(This is like how the Terms of Use Section 4.4 prohibits uploading projects made using TurboWarp, yet the Scratch Team doesn't usually take action against projects that do this, to my knowledge. Or how copyright holders can submit DMCA takedown requests against your projects for using their copyrighted material, even though many copyright holders don't go through this process.)
