“Your Worst Nightmare” from Undertale. Gosh, that song scared me so much…i said that on a scratch post and some rude guy said UH NO ITS NOT

Anything from the Swans albums Soundtracks For The Blind and The Seer. Only songs that don't terrify me on either of those are “A Piece of The Sky”, “Song For A Warrior” and “The Sound”.
A good chunk of rest of the Swans discography is worth mentioning too, especially the song Oxygen. It seems to have been written purely to make the listener exceedingly uncomfortable.

The only really creepy bits of music I can think of off the top of my head are typically at the end, such as “My Poem About Frogs by Cliff Evans” by Bootsy Spankins P.I. and “Take My Hand” by Matt Berry. That said, on what I think was a separate topic with a similar theme, I did suggest Revolution 9 which seems to have popped up here.

Search up “Super Paper Mario OST - River Twygz Bed”. It's creepy, especially with the creepy distorted whispering saying things like “SAVE US”.Yeah, it was creepy… The hands and the atmosphere of the level got to me and I was really scared. (For playing a game, which isn't that bad.) I didn't know about the distorted whispers though.

Also, the Game Over music from Sonic Adventure gives my shivers.
The “picture music” from Sonic CD is actually the boss theme, which is why the music for the picture is different in Japan and Europe and sounds more “edgy” than creepy
