Is The Coffin of Andy and Leyley appropriate for Scratch?
The Coffin of Andy and Leyley is a horror game about a sibling duo who escape a “quarantine” where they are being starved. My main issue with the game being discussed on Scratch is the cannibalism (and, subsequently, relatively graphic visual gore) the characters participate in for both survival and revenge, and the large number of suggestive scenes, as well as the very toxic, heavily implied to be romantic, relationship between the two main characters, who are siblings.
Specifically, my question is for if projects containing scenes from The Coffin of Andy and Leyley and profile pictures containing content from the game are reportable.

Based off of what you said of what's about the Coffin of Andy and Leyley, it's not appropriate for Scratch. Though projects of it (if it doesn't contain any of the horror material from the horror game) can be allowed.

Seeing how it seems to have proshipping elements(ships that shouldn't be normalized/glorified, such as abuser/victim, blood relatives, etc), I wouldn't recommend mentioning it.
If there were just violent scenes that can be overlooked(in more tamer levels/cutscenes), I would just say “just avoid mentioning the bloody stuff”.
But, that stuff, that appears to be glorified….

scratch team will probably get your project “shadowbanned” from search.no, NFE is saved for things on the border of being inappropriate for the site. they're ontop of a line they shouldn't cross - they aren't past it, but aren't behind it, either.
but likely wont get reported.
it will get taken down more likely than NFE'd imho.
even then, https://scratch.mit.edu/contact-us/ is the only party that can give a definite answer over this.

But if it's just like an image of one of the characters with nothing against TOU, can they really take it down or NFE it for referencing explicit content? add on to what I said above, quote from the TOU, “Contains sexually explicit or graphically violent material;”. Keyword is ‘contains’. The question is if it uses ‘contains’ in reference to the project itself or more…

Yes, this is what my original question was trying to say: if even appropriate photos from the game put on Scratch are reportable as a result of the entire game's story.But if it's just like an image of one of the characters with nothing against TOU, can they really take it down or NFE it for referencing explicit content? to me it sounds like a case like that one 18+ hotel animation thing (that i forgot the name of), but again, i'm merely speculating here based off something i think is similar to some degree, atleast from the given descriptions.

While normally M-rated fandoms are fine to talk about so long as discussion of them remains Scratch-appropriate, I feel as if the relationship you mentioned is inherently bad enough that you're better off not mentioning the game in question on Scratch at all, especially when you consider that it's a pretty glaring part of that game's 2nd chapter and young children do not need to know about Sweet Home Alabama™ under any circumstance.

After reading the topic, alongside getting info on the game from a gameplay vid of the recently released Episode 3, I'd say projects related to it are allowed for the exact same reason why projects related to OMORI and Homestuck are allowed: As long as you don't address the parts about the game that could violate Scratch guidelines such as the horror and, let's call it the “special” relationship between the two main characters and leave it at that, you should be fine.
TLDR: Talking about this game should be fine as long as you don't speak of the “special” stuff regarding it on Scratch.
