You didn't save it, right?

This happened to me too, and it's just saved as that… it autosaved.

the bug was acknowledged by scratch team, they will try to fix it: https://scratch.mit.edu/discuss/topic/503423/

I don't know if this will help, but your device might need more RAM to load the assets. And try saving using the “Save to your computer” feature after you're done editing, and if costumes go question mark again, try loading that version.

I would also always manually save and download before I leave the page.

it's the image file that the problem you are probably using the wrong damage file.

When this happens IMMEDIATELY CLOSE THE TAB! DO NOT SAVE!
