Review Form:

Employee Leave Form:

Rules:

Customer Rules:
- Give enough time for the order to be done
- If you are not satisfied with the result please say so kindly and say why you aren’t satisfied
- Put your favourite kind of tree in your form to prove you have read the rules
- Please leave a review
- If you wish to repay the person who did your order then follow them

Staff Rules:
- Be kind
- Reply to activity checks (will be held every month)
- Please carefully follow order description
- Put the your favourite colour in your application to prove you have read the rules
- If you want to be Co-Owner or Organiser and the spot is open please say so in your application
- I will invite all staff to The Ultimate Thumbnail Shop Staff studio and promote them to manager for the main studio
