My guess is that you're using vector mode. Vector mode only fills in the fill setting of a shape, not the outline, so if you freehand, you're out of luck for the fill tool. If you convert it to bitmap, it will work regardless of the circumstances, but be careful. Small gaps in the drawing can lead to the entire thing being filled one color.
