Somebody mentioned “the crazy one” who lives in a cave. Maybe the player has to complete a quest before getting some info (what info, ideas pls) about his sister's whereabouts.

After the quest, the crazy one tells him to find the ________?

Maybe:
MC is looking for sister, and some village folk speak of a strange, crazy man who lives in the cave systems nearby, who constantly speaks “oracles”. MC decides to see the crazy man for himself but gets attacked for some strange “energy/essence” inside of him. After battle, the crazy one regains some level of coherency and tells yet another oracle to the MC, who then moves on.

The message can be some form of poetry with Latin and old English entailing some vague things about the twin. Not enough to tell the MC where the girl is, but just enough to let him know that she exists, had been spotted by the crazy guy, and is somewhere.

As a game developer, I'm going to put some thought into whats happening (since we have the main idea down). It can start in the forest area, going to the desert. They need to find a jar of sand in some cave (this links to the crazy man story, they think he can help with the sister and might have sand) and, once they have that, they need to find the village craftsman/blacksmith and make a key with a certain template found with the sand. Maybe they need to separately look for the mold/template.

I like the idea! I think that once they find the crazy man, they should be able to continue, so not separately find the mold. I think we should continue through the three realms as fast as possible.
