Scratch 0.1 isn't lost, you can find it here.
There was an earlier 2003 build that was lost.
The scratch wiki says it's the first publicly available build of scratch, if we want to find earlier builds we must steal the scratchteam's hard drive.

A .image file?
That's not an archive, renaming the file to img or iso (assuming it was a disk ‘image’) didn't work neither did png or jpeg ('image)

It's always best to use standard, and common image formats. That can be opened by default in the operating system everyone uses that is the best (Windows)
disk image: ISO, IMG
image image: png, jpeg, bmp, etc

Isn't .image used for squeak?

Yeah, to be clear, “image” doesn't refer to an image as in picture, it refers to a squeak VM image. Just drag that file on top of any squeak VM (including the Scratch 1.4 EXE file) and it'll open up.

Alternatively if you can't do that, you can run Scratch 0.1 online without downloading anything by going to https://scratcharchive.naleksuh.com/?run=Scratch14Feb04.image (note that it will run a bit slower here due to the browser emulation, and that it will only run as intended by downloading)

Well to get the Scratch 1.4 EXE to open it, but Scratch 1.4 you sent was IN an .image file. How do I extract the contents?

You don't.

The .image file is not to be confused with .iso files
It is a Squeak image and you run it in Squeak vms (like the Scratch 1.4 EXE)

You don't. Also, I don't think Naleksuh sent Scratch 1.4. To get Scratch 1.4's EXE you can always just download the Scratch 1.4 installer, install it (or extract it if you're moist ) and drag the Scratch 0.1 .image file into Scratch.exe.

Scratch 1.4 had a change to the Squeak VM, so if you want to run that image I'd recommend you use the 1.3 VM or earlier. Grab that here: https://download.scratch.mit.edu/ScratchInstaller1.3.1.exe
