Hey there, of my time on Scratch I have found out that to develop an extension (which I assume is what you are asking) you need to read this article at https://github.com/LLK/scratch-vm/blob/develop/docs/extensions.md it will help you understand how to create one and also how to get them on to Scratch as well.

I am stil confused

Basically you cannot create an unofficial extension without having permission from the ST to implement it into Scratch, what programming languages do you know anyway? Extensions are complicated (sometimes) to make?
