Pixels. If you move your mouse around the scratch editor there are two numbers that show the coordinates of your mouse on the stage on the bottom right corner of it. When importing images to Scratch 480x360 pixels will take up the entire screen without anything being resized, however when exporting things from scratch, scratch will take sub pixels into account making the image twice as big.
